Adult Wellness Plans allow you to receive essential preventative care at a low monthly cost! This plan will guide you through 4 quarterly visits for your pet throughout the year.

Visit 1 – Annual Vaccines (Rabies, DAPPv, Bordetella, and Lepto), Heartworm Test, Tri-Heart Plus 6 month, and Bravecto (flea/tick prevention) 3 month

Visit 2 – Bravecto (flea/tick prevention) 3 month (Refill) and a fecal test

Visit 3 – Bravecto (flea/tick prevention) 3 month (Refill), Tri-Heart Plus 6 month (Refill)

Visit 4 – Bravecto (flea/tick prevention) 3 month (Refill) and a fecal test

The sign up fee is a one time fee applied to the 1st month’s charge. The fee is charged because the first month’s visit includes a large number of services/ products.

I understand that these plans are for mature pets. They are not wellness kits for puppies or kittens for their first year of life.*

I understand that if my pet's weight exceeds the maximum dosage of Tri-Heart Plus (100lbs) or Bravecto (123lbs), I will need to purchase the medication needed to provide proper coverage for my pet's weight on the day of pick up.*

I understand that additional vaccine boosters may be recommended based upon my pet's individual vaccine history and that these boosters are not included in the adult wellness plan subscription.*

I understand that I will not be refunded for services my pet has received.*

I understand that even if I cancel my pet's plan I still may owe additional fees for services my pet has received before cancelling the plan.*

I understand that heartworm positive pets do not qualify for Adult Wellness Subscriptions. If my dog is heartworm positive this subscription will be cancelled and refunded. I will be charged instead for only the services received on the day of my initial visit.*

I Understand that I must pick up my refill of heartworm prevention within 7 months of my pet's heartworm test. If I do not I will be assessed a $20 fee for a new heartworm test before I can receive the refill medication.*
